Person-Centered Care

The goal of Person-Centered Care is to discover the kind of life a person desires, create a plan for how to achieve their vision, and identify ways to access needed supports and services. The focus is always on the person’s vision for who they’d like to be and what they’d like to do — planning centers on strengths rather than weaknesses.

The process brings together people committed to helping someone articulate their vision for the future. Those who care most deeply about the individual form a team to contribute ideas, solutions, connections, and action steps toward their goals.

Person-Centered Planning allows individuals to build a path toward the life they want, giving them control over their own decisions and helping identify who’s involved in their life. Plans are designed around the individual, with the goal of making the process accessible to them and the people around them.
